All new clients need to be screened and verified before accepting appointments. Please provide one of the following  information along with your full name and telephone number.

1. Two recent (within the last six months) established provider emails only, no phone numbers

2. P411 members, please send a pre-screening or an appointment request form via the P411 system

3. Employment Information: 

   picture of your government ID along with your business card, side by side, showing matching name, along with a blank email from your listed work email.

 

 

Once you are verified, I will get in touch with the meeting details.
